It’s Social Prescribing Day and we’re sharing Tracy’s story to highlight the great work our team does with GP practices across Kirklees.
Meet Tracy
Tracy lives with her sister (who is also her unpaid carer) and has been unable to work due to her mental health and medical condition. Both women support each other as Tracy’s sister also has health challenges.
Tracy wasn’t having a great time – she suffered from chronic pain, fatigue, anxiety and wasn’t sleeping well. Her mental health was also impacted by historic abuse in the past and as a result, Tracy didn’t want to travel on her own – somebody always accompanied her as she needed support to be able to leave the house.
Then she met Sarah…
Help from Social Prescribing
Tracy was referred by her GP to Social Prescribing Link Worker Sarah who worked with Tracy to bring about some key changes in her life. Sarah supported Tracy to identify the reasons behind why Tracy felt she wasn’t listened to. They also worked together to make sure Tracy could get the most out of her appointments with professionals. They chatted about building confidence, pain support groups and mental health support for the abuse Tracy had experienced in the past.
Onwards and upwards
Over a period of 3 months things began to improve for Tracy. She was more confident; felt valued and listened to; increased her physical activity and exercise levels and was better able to communicate during appointments. She was catching more zzzz’s because of her new pain medication and was able to go out and about on her own.
In her own words:
“I have a lot to thank Sarah for, she helped me find my voice, how to communicate and ask for the correct help that I desperately needed. When someone actually listens and hears you, it helps you move forward. My mental and physical health has greatly improved and will continue to improve with the help, support and advice given by Sarah and the team.”
